We provide a more bespoke service to those with disabilities or who may require additional assistance with their eye examintion. Before attending your eye examination please send us the completed form and this will help us to understand your needs, and provide you with the best possible service we can.
This is the form for you and your supporters to download and fill in before your eye examination. Simply fill in the form and send it back to us, either by post or email. Your answers will help the optician understand your needs, so using this form will help you get the most from your eye examination.
Remember
- It’s a good idea to give information to the optician before your eye examination
- You don’t need to be able to read or talk to have an eye examination
- You should have an eye examination every two year
Why do we need this?
It is likely that some of these patients will be missed if they do not have proper access to eye care services. A standard eye examination is not really tailored for the needs of someone with a learning disability and there may be better way of doing things.
This is why all of our staff, including reception staff, are trained in-house to work positively with patients who have learning disabilities. If you would like to find out more about this service, please contact us at your earliest convenience.
